When a significant problem reaches a customer — a batch of failed parts, a recurring defect — they often won't accept 'we fixed it.' They want to see a disciplined investigation. 8D (Eight Disciplines) is the structured, team-based problem-solving method used for exactly that. In Flaggity, 8D is a type of CAPA: choose it and the standard form is replaced by a guided D0–D8 workflow that produces a polished, customer-facing report.
When to use 8D
Use 8D for problems that are serious, recurring, or customer-facing — the ones where a one-line corrective action won't be credible. For routine internal non-conformances, the standard CAPA workflow is plenty. 8D is the heavier process you reach for when the stakes justify it.
The eight disciplines
- D0 — Emergency Response: immediate action to contain the impact right now.
- D1 — Team: name the team, the leader, and the sponsor who'll see it through.
- D2 — Problem Description: a structured, quantified statement of the problem, including an Is / Is-Not analysis that pins down what is and isn't affected.
- D3 — Interim Containment: stop the bleeding while you investigate, so no more bad product ships.
- D4 — Root Cause: a guided 5-Whys analysis, including the escape point — why the problem wasn't caught.
- D5 — Permanent Corrective Action: select the real fix and justify why it's the right one.
- D6 — Implementation: put the permanent action in place and verify it's actually effective.
- D7 — Prevent Recurrence: update systems, procedures, training, and similar processes so it can't come back through another door.
- D8 — Closure: capture lessons learned and get management sign-off.
Reports and NCR linkage
When the permanent action (D6) is verified effective, a linked NCR updates to show the corrective action was implemented. On qualifying plans you can generate a professional 8D PDF and send it to a customer with a verification link, so they can confirm the report is genuine — which is often exactly what they require to close the issue on their end.
What happens next
A completed 8D gives you two things: a customer-facing document that demonstrates real rigor, and a permanent internal record of the investigation. Once D8 is signed off, the 8D lives in your quality history alongside its NCR and CAPA, ready for the next audit or customer review.

Is 8D overkill for small issues? Yes, often. That's why it's a choice, not the default. Use the standard CAPA for routine issues and reserve 8D for the ones that warrant a full team investigation.
